Financial Review (Phase 2 – By Invitation Only)

If selected for Phase 2, applicants must submit the following documents within 3 weeks of the request. Failure to meet this deadline will result in automatic disqualification from funding consideration. Required: IRS Form 990 (Last 2 Years). Strongly Recommended: Balance Sheet (Last 2 Years) and Statement of Revenues & Expenses (Last 2 Years).

Final Certification & Agreement
